PLEASE TURN OVER &
SIGN PARTICIPATION CONDITIONS
Conditions
of participation
1. Parents and players agree to be bound by the rules, regulations and policies of St Peters Iver FC as amended from time to time. These will include but not be limited to the parent & players code of conduct. Full details are available on our website at www.spfc.co.uk or on request from any of the club officers.
2. Training will begin promptly at 9.30am and normally last for 1 to 1˝ hours. Parents must ensure that their children arrive prior to the start of training and that they are collected promptly.
3. Parents/guardians of all children participating in club activities (training & matches) accept that it is their responsibility to sign the child into and out of each activity. Managers & coaches will not allow participation in, or release children from, the activity without an appropriate signature.
4. Any child suffering from asthma or any other illness that may require medication must bring the required medication with them each week. Parents are responsible for advising the coach/manager of any health matter that may affect their child during either training or a match.
5. Parents must ensure that their child/children are properly equipped for training and matches. The following are considered essential:
a. Football boots should be worn. Trainers are sufficient for under 6’s.
b. Shinpads must be worn for training and matches.
c. Children should always come with sufficient warm clothes. Remember layers can be removed!
d. A drink, not fizzy, should be bought to all training sessions and matches.
6. Children representing any of the St Peters Iver teams should arrive at the time requested by the manager. If it is necessary for your child to arrive early, i.e. if a sibling is attending training, please ensure that they wait quietly until their manager arrives.
7. Parents/guardians of children representing the Club must be aware and agree that it is their responsibility to transport or arrange transport for their child to and from all away matches. Sometimes, for convenience, we may meet locally prior to an away match. The responsibility of St Peters Iver Football Club in these instances begins only when the players arrive at the destination and ends once the match is finished.
8. It is the responsibility of all parents/guardians of team players to ensure that their kit is returned when requested. This is especially important if the player is not available or not selected for the next match. It is the responsibility of the player’s parent/guardian to deliver the kit to the manager prior to the next match.
9. St Peters Iver Football Club aims to provide a friendly, enjoyable, encouraging, sportsmanlike and disciplined environment for children to develop and participate in soccer. We request that parents/guardians and children respect this philosophy at all times and behave in a manner appropriate to achieving this aim.
10. The information given on this form may be retained on a database and used in the day tom day running of St Peters Iver Football Club. St Peters Iver Football Club will take reasonable care to keep your personal information secure and prevent any unauthorised access or unlawful use of it. We process all personal information in accordance with applicable UK data protection legislation.
11. From time to time parents or spectators may wish to take photographs of their child or children participating in the match, training or other St Peters event. Team photographs may also be taken. On occasion these photographs may be used in the parish magazine or on the club website. You agree that you have no objection to legitimate photographs being taken which may include your child or children.
I have read and understood the above notes and accept them and agree to be bound by them. I accept that it is my responsibility to inform the Club directly of any changes to the details recorded on this form.
